       Physician Certification Form

       The attached form is a copy of version 4 of the form A Code
       Status Attached  checkbox was added to indicate that DNR
       paperwork was attached. An Other box and ALS/BLS check boxes
       have been added. The rules for using the form and previous
       versions of the form follow:

       [li]All fields must be filled out by the originating institution
       except for the EMS ID # which will be the Incident Number.[/li]
       [li]RN Signature is not needed if the physician prints their
       name and signs the form.[/li]
       [li]It is your responsibility to have the originating
       institution fill out all fields. Do not fill out any fields
       except for EMS ID #.[/li]

       I was not aware of these "new rules" when the "errors" occured.
       Maybe you should make us aware BEFORE you consider these errors.
       This was definately not a policy before today.
