       After a recent rewatch of JSS, I noticed that Carol was looking
       at the cigarette thinking about smoking it, which I believe was
       the beginning of her downfall then but at the time she was still
       resisting in giving into it.
       Then when we see her much later in "Not Tomorrow Yet" she's
       given in and starting to smoke the cigarettes in order to deal
       with the pain she feels. It's almost as if she's punishing
       herself too in a way. She told that woman those things would
       kill her and yet here is Carol smoking them now, so she must
       have thought about dying at that point.

       I agree that Carol's smoking is her way of dealing with the
       guilt (stress might be a better word) of having to kill people.
       I read a fan's blog in which she hypothesized that in season 6B,
       Carol was punishing herself by smoking, her brief relationship
       with Tobin, and avoiding the group i.e. Daryl.
       But, when Carol and Maggie were captured in "The Same Boat", she
       used some psychology on her captors: she made them believe she
       was a mouse, when in reality, Carol was willing to kill them.
