You shouldn't even have that.
It's pretty random item, it can heal, give vitality, give lives,
give honor points, boss points, etc, etc. It's like Pandora's
Box.

I version 1.5 the puzzle has been disabled only way to get it
should be to modify campaign in editor.

Actually it was somehow possible to have it in 1.4c, even if the
puzzle bugged.

I used save/load to solve the puzzle. Game version is 1.4c.
After using ?trial and error?, I realised that if you save and
load the game after you run around to find the source, the code
does not reset. So, this is what I did:
1 activate a source, find the source (ignore the hints spamming
across the screen), run back to the puzzle box and save before
you try to solve it.
2 enter ?-000?, ?-111?, ?-222? .... ?-999?. If it says
?something is right? on ?-xxx?, then you?ll know the number X is
in the code. Keep doing this 10 times (worst case) and you?ll
know exactly which 3 numbers are in the code. If your tries run
out, just reload and try the remaining numbers.
3 try different combinations of these 3 numbers.
You can solve level 1-2 puzzles without reloading, level 5 will
need few reloads as it only gives you 3 tries.
