How do I add a Favicon image to my site

I have tried but it couldn't work please explain to me how i can
add a favicon image to my forum.

Please admin help me out. I tried to add the favicon icon to my
but it didn't work. Bellow is what i added to the header of my
forum please what else can i do or you can drop me a code dat
works.

How can i upload an image to my forum if not through attachment
so as to get the image url not attachment url.
And should this <link rel="icon"
type="image/png"
href="my image url name"> be added to the header or the meta tag
