       Forms on the forum

       Is there a way, most likely thru the page manager, where I could
       add a form that would be filled out by a person visiting the
       forum? Let's say for the purpose of requesting a service or
       something like an order form. This could be customized by the
       forum's admin to their specific needs.

       your email?
