       Subscription, a copy of their ordination certificate, and a
       letter including:

       Name
       Home Address
       City State/Province Postal Code
       Country
       Mailing Address
       (If different than Home address)
       City State/Province Postal Code
       Country
       Home Phone Mobile Phone
       E-mail (Personal) Spouse’s Name
       YOUR CHRISTIAN SERVICE:
       Place of Service Position/ responsibility:
