The Law - at large - has a principle of de minimis, insofar as
that it does not concern itself with trifling matters. Football
isn’t all that different.
ARs have two jobs - be aware of the throw in position/action and
whatever is going on on the pitch. You can only really focus on
one.
It’s a combination of priorities, and not getting hung up on
throw ins which may be partially illegal (e.g. second foot
lifted slightly off the ground, front foot slightly over the
whole line, ball not quite released correctly).
For foul throws, you only want to be cracking down on egregious
examples. Tiny infractions invite criticism of being officious
and ‘making it about them’ and certainly don’t help with match
control.
See also: throw ins and free kicks taken from the position of
award. Again - trifling differences, we don’t get hung up on.
Edit: by all means feel free to disagree, but that’s basically
how it is. The game at the very top level is coached on match
control as much as it is on accuracy. You don’t manage top level
games for searching for every tiny infraction - that gets
coached out of referees a long way short of the Premier League.
Just look at the Arsenal - Bayern ‘penalty’ debate from a couple
of seasons ago when the referee called the claims for handball
“a kid’s penalty”. Unless a throw-in is egregiously wrong, they
don’t get called. It’s not kids football.

I can assure this is done at step 4 and step 5 where I officiate

hi,
recently, it's not just the feet placement but also the 'from
behind and over the head' that is being missed - from an arsenal
point of view, riccardo calafiori and myles lewis-skelly are
serial offenders [emoji16] also the amount of time routinely
taken to get the game going again is exasperating [emoji34]
likewise the 'stealing of a few steps down the line' seems to be
worse than ever [emoji15] and finally, the current trend for the
long throw into the box can have lengthy delays whilst a towel
is found, the ball is dried, the penalty area is loaded etc.

When you say feet placement, do you just mean where feet are
past the touchline, or do you also mean feet leaving the ground?
If the former, and the Palace throw-in you highlight was the
only one where the entire foot was past the touchline, then that
would have been the only 'foul' throw missed!
As for the second question, I believe it should be within VAR's
remit, as it would be an offence by the attacking team in the
build-up to the goal.

In each case, the leading foot had no part on the touchline or
on the ground outside it. I hope VAR check long throws

I didn't think that checking offences relating to the restart of
play was within the VAR remit. I recall one a few years ago
(possibly Spurs at Old Trafford) where the attacking players
were in the opposition half when a kick off was taken (and a
goal subsequently scored) but that wasn't overturned.
