Being intentional doesn't make it a cautionable offence.

Off-hand I can't say I remember a player being cautioned for an
offence like this - I vaguely recall a few where they were
cautioned as it was stopping a promising attack though. I'm sure
I've seen other players this tournament penalised for
holding/pulling offences without being cautioned.

Not explicitly, but intentionally holding an opponent is as good
an example of unsporting behaviour as one can find. You cannot
use hands to tackle / challenge so doing so is an intentional
and pre-determined act.

As you stated, holding is never accidental, so by that logic any
and all holding offences are cautions? Behave!
