Most referees normally stop play, especially when they have a
card in their hand.

Reward the offended side - usually gets called back for ball in
wrong place
Goal is a good one

I wouldn't be allowing the quick free kick once I've got the
card visibly out.

I agree, I would have expected a whistle to kill the play there.
I know it can frustrate the attacking team but if defenders have
seen him get the card out there, they will be switching off and
setting up to defend rather than reacting to the quick one

And the assistant on that side could have easily switched off
having seen the card come out.

But you’ve just contradicted yourself. The referee is clearly in
the process of cautioning the player and is taken miles from the
wrong place.
