issue a charge against the offender? Or are we then straying
into the realms of re-refereeing the game?

Yes, I think they should step in and no, in my opinion, it's not
re-refereeing the game. My understanding is, in cases where the
Match Referee hasn't seen a serious incident, the FA can (and
has done so many times in the past) take retrospective action.
However, if the Referee were to say he/she saw the incident,
then the FA cannot intervene as they deem that the Referee dealt
with the matter on the day.

The FA posted an hour ago on there Twitter feed that
Thomas-Asante has a 3 match ban for violent conduct, match
officials missed it but caught on camera

If match officials miss an incident like this is there likely to
be any repercussions for the officials concerned?

Only if observer believed they missed it due to poor positioning
or lack of identification.
Given where the ball was RW and active assistant had no chance.
Pictures give no indication where other AR and 4th are, so
cannot comment.
They will be upset to have missed it but should not adversely
affect marks too much.
