May I know if customer paid us extra Rm5.00,but our company
decided to recognised this RM5.00 as income. So is it need to
account for GST output?

I spoke to a GST Consultant
Answer given:
Best to raise a Tax Invoice for the excess money received
Tax Base+GST = Excess money received

Thanks for reply.
I also got call Custom to ask this question. He said our company
do not supply of goods or services for this extra money. Thus no
need issue tax invoice. But I worried custom will question us on
this matter in future if we do not raise tax invoice due to I do
not have the proof on conversation.
