I still feel protocols should be estabilished what if sword goes
rogue, who stops them, or what if hammer reforms with all
villains working together, who stops this?

S.W.O.R.D. itself is the protocol. An entire organization has
oversight, no individual can do what they want unless they get
approval from other people.
And if HAMMER reformed, Abigail would have S.W.O.R.D. stop them
because she would have S.W.O.R.D. be in charge of stopping
anyone ever.

know an bring in outside parties to form a multiversal shield
group an reincorporate all it divisions, an bring it back as one
unit

S.H.I.E.L.D. had divisions for a reason, merging them all would
just decrease the oversight that the divisions were created to
accomplish.
And if there is a multiverse they're aware of, they would by
default have to patrol that as well. Though, if every universe
had a S.H.I.E.L.D. patrolling the multiverse, how would they
ever decide which S.H.I.E.L.D. was in charge?! ???

No, because then no one would be in charge of them.
The aim of government is to have MULTIPLE bodies each hold one
another to account. If S.H.I.E.L.D. could go anywhere it would
be entirely up to them what can and cannot be done, which begs
for abuse of power when you can't guarantee the mindsets of the
people in charge, like how HYDRA brought the entire organization
down in the Marvel Cinematic Universe.
