Child Application (ages 4-12)
We ask anyone interested in joining us to fill in an application
and send it to: homecoming.point@gmail.com
Parts one and two of this application will form your 'member
profile' if accepted and refer to the inner applying to the
board. Part three is 'real life' information for the reference
of the Administration only. You must fill out and submit every
section of this application to be considered for membership.
PART ONE

and discipline here? If you aren’t coming with anyone, write
N/A. We’ll assign you to a foster home, and tell you who your
care worker(s) is/are. Your care worker(s) are the adults
responsible for you.):

PART TWO - The Big must fill this out
Please describe the amount of discipline the child would need in
their relationships:
Who may administer Discipline? Please select one of the
following. Note: Must have an adult’s permission to list them
under #1.
(1) Discipline may only be administered by the following people
(must be on-site and active).
(2) Any adult may discipline the child.
Discipline and Guidance Information [If something is a limit or
a trigger, please say so. Please give detailed information as
this is how we know what will work best for your child] -

2. Being sent to their room for a time out?
3. Being put in a chair for a time out?

Other forms of effective discipline:
Are you comfortable with men disciplining the child?
Are you comfortable with women disciplining the child?
Are you comfortable with others changing, bathing and dressing
the child? Be specific about their needs and likely response:

Please list what the reaction will be to a trigger, and how it
MAY be handled (not necessarily will).
Please take the time to list ALL hard limits. Hard limits are
triggers that may not be approached or broken for any reason.
