       Request an unban
       1)Nickname you used when you got banned:
       2)Name of the admin who banned you:
       3) Reason:
       4)Kind Of Ban : Perma ? or temborarily
       5) Proof/Reason why you should get unbanned: .demo of the
       gameplay when u got banned OR a video of it.
       6) Name of the server and platform u got banned (mw3 or mw2 or
       bo2) ?
