       -Have you ever been banned/Disabled:
       -If Yes, for what reason:
       -Why do you want to join us:
       -Your real age:
       -Any of Your Friend's Name, that are in GayZa, DoT, or any other
       enemy group?:
       -A screenshot of your /sts and /msts at LV:
       Also at last: If you have a montage or a  gameplay video to show
       us your aiming skills, post your youtube link together with ur
       application.
