Standard Issue Character Profile Sheet

I've spent the last couple of days designing and testing a
character sheet for use by all members of the Relayus Novum and
their primary RP characters. What I mean by primary, is the one
we associate with our usernames - for example I would have Meros
(and possibly So'rem) as my primary Characters. Muffin has
Muffin the Heretic as her primary character and Senera2000 has
Senera the Black Templar as his.
If you really wanted, you can use this for any character. But
due to the length of this sheet and the detail it will require
to fully complete, it's hardly worth the effort for a small, one
use character that you'll probably forget existed in a few weeks
time.
In the fist spoiler tag is the profile with descriptions for
what exactly goes in each section. In the second spoiler tag is
a blank profile without the description and just the headings so
that it can be easily copied and pasted for your own use.

Name: The full name of your character, including middle names
but excluding any titles such as ‘Doctor’ or ‘Captain’
Species: The species of which your character belongs to, such as
‘Human’ or ‘Ork’. Also include if your character is a mix, such
as ‘Human-Eldar’. Beings of technically the same race, but may
be a different 40k codex (such as Craftworld and Dark Eldar) are
to be listed as their separate races as determined by the codex.
Gender: There are TWO genders. I don’t want any of the bullsh!t
you bring from Tumblr infecting this profile. Although, for the
case of species such as Orks which have no defined genders, I
will accept N/A.
Sexuality: Simple. Do you like c0ck, or t!ts? Not hard… well,
one of them can be hard but that’s not what I mean…
Place of Origin: Not looking for an exact address, unless you
want to be that specific. Something like the planet, and maybe
where the planet is in terms of galactic Segmentums would be
suitable (or not, if you decide on living in the warp, or
perhaps on a ship drifting from place to place.)
Approx. Age: Preferably in Terran years.
Build: Burly, slight, strong, frail, whatever. Always use in
comparison to the species. So, if you have a ‘frail space
marine’ then he’s still going to kick the shit out of an Eldar
in a fight, but maybe not as fast as a ‘burly space marine’.
Allegiance: Do they worship the Emperor, or are they a scion of
the chaos gods? Maybe your Eldar character hails from Craftworld
Lugganath, or lives under the deceitful eyes of Asdrubael Vect.
Does your fire warrior bow to the Ethereals, or follow Farsight?
Whoever they fight for, put here.
Type/Rank: Scrub or Sergeant? Corporal or Commissar? Fire Dragon
or Farseer? You get the point…
Facial Features –
a)
so a fat character will have a fat face, maybe a double chin
too… or more.
b)
over the face and head. Even nose hair, if you want to be that
disgustingly specific.
c)
the eyes such as a bionic then that goes here too. If their
facial structure affects the eyes, such as making them appear
larger or smaller, put that in section ‘a’ of Facial Features.
d)
eyes in section ‘c’) as well as other aspects of their face that
don’t fit elsewhere.
Body Features: Include items like skin tone, any deformities,
disabilities or mutations whether occurring at birth, a result
of wounds or whatever. Maybe a lost limb is replaced by a
bionic, if so tell me here. Scars, burns, birth marks – stick it
in this section.
Mental State: Krazy like Khârn, or calm and collected like a
meditating Farseer? Or somewhere in between like a guardsman who
can’t decide if he’s terrified or the biggest hero the Imperium
has seen since Ciaphas Cain.
Relationships – This section is mostly intended for the names of
other individual characters. Although in some cases, an
organisation or an entire race is applicable.

people any day of the week.

clear what family member each person is. We wouldn’t want
someone confusing your mother for your sister.

of company, but they wouldn’t be disappointed if they were.

must die by your character’s hands.
Clothing: The average, day to day clothing. Multiple outfits can
be written here, but please not the entire wardrobe.
Armour: Do they fear each coming day wearing standard issue
Astra Militarum cardboard, or wear a swanky suit of power
armour?
Gadgets/Wargear: Items that would be classed as wargear in a
Warhammer 40,000 codex are included here. Such as Jump Packs,
Signum links, shield generators and other bits of tech your
character carries around.
Weapons: All Dakka and Choppy is to be listed here.
Owned Transports: Maybe they drift on a motorbike from town to
town, or travel in luxury aboard a behemoth sized starship.
Personal Motives: Why is your character here? What are they
after? What do they seek in life?[/spoiler]

Sexuality: Bisexual but does not care about having sex
Place of Origin: Orphan, grew up on Ophelia

Build: rather strong for a human
Allegiance: "I love the Emperor, but Slaanesh has granted me
many things." - Muffin
Type/Rank: Space Pirate?
Facial Features -
a) Facial Structure: roundish face but the cheeks are inward
due to malnutrition
b) Hairstyle, colour and facial hair: used to be brown for a
while but it turned pink and purple uppon corruption, it's
shoulder lenght except for some at the side of her face which is
longer. Irregular Bangs.
c) Eyes: Cyan (the pupils turn pale durring illusions or mad
fits)
d) Misc: two purple, sharp horns grow out of her head, both on
the right side
Body Features:
Horns, as mentioned.
Her right hand was burnt and mutated into purple tentacle
fingers.
Scars all over the body, but most of them are hidden by cloth.

Name: Unknown... although for some reason everyone has been
calling him Norabder, this still confuses him a bit.
Species: Human; Astartes
Gender: Male
Sexuality: Straight cisgendered oppressor ( :P )
Place of Origin: planet recruited from; Unknown
Approx. Age: Unknown
Build: slightly above average strength for an Astartes
Allegiance: loyal Imperial; Lamenters, blood angels and
Sanguinius.
Type/Rank: Unknown (could be anything from battle brother to
captain)
Facial Features –
a) Facial Structure: appears gaunt and stern, no scars or
other marks on face.
b) Hairstyle, colour and facial hair: short blonde hair, clean
shaven
c) Eyes: blue
d) Misc: left arm is bionic and built into the armour, as such
the left arm section of armour cannot be removed.
Body Features: scars around the left side of the torso as well
as one scar just to the right of the small of his back.
Mental State: normally calm and reserved but will quickly take
command in a combat situation.

c) Close friends: Cpl Alberto Malich (formerly Valian 2nd,

Weapons: boltgun, combat knife, infernus pistol.
Owned Transports: none (although has command of an Imperial
guard Vendetta on loan from Vale Primus
Personal Motives: discover the fate of the Lamenters, kill
Minotaurs and tyranids...
and once that is done purge the xenos and burn the heretics. (so
nothing much just kill a few billion life forms...)

Sexuality: "Not applicable... well, actually... I had a thing
with a Sororitas once but that was a long time ago. She was best
friends with this half metal Blood Raven and- ... ok, sorry.
Please continue."
Place of Origin: "The sands of Baal Secundus."
Approx. Age: "My body is no longer than a couple of decades in
age. Although, I myself have been living for at least ten
thousand Terran years."
Build: "I'd say rather average for a Space Marine."
Allegiance: *sarcastically* "For the glory of Sanguinius and the
Emperor. And possibly Slaanesh, but I try not to get too
involved over there."
Type/Rank: "Apothecae Minoris."
Facial Features – *The recording features the sound of a helmet
being removed. The voice of the interrogator takes over*

everlasting smirk." *He chuckles*

length-" Actually my hair is blond. I just like to dye it on

someone stands* "Arms out please... ok: pale skinned, small

collarbone. Left arm is completely bionic, and appears to be a
mixture of Mechanicus and Xenos technology."
Mental State: "I'm naturally quite an angry individual in the
thick of a battle, but considering I slaughter entire
populations of people for a living I reckon I'm rather sane."

once did it with a Farseer, but I'd drank far too much Fenrisian
Ale to even begin going into details."

definitely didn't steal."

the knowlege that the Mechanicus won't shoot me on sight."

subject name: Segovax or the "red butcher"
Species: Human for the most part
gender: male
sexuality: has lost the preference since the indoctrination into
the world eater legion
origin: Damitos- Ultrama sector
age: unknown, Centuries?
build: from a combination of the space marine enhancements and
the world eater's more... brutal enhancements has left him a
bulging muscular mess
allegiance: Khornate
rank: Leader of a warband of traitor astarte pirates
Facial features: covered in overlapping scars over sickly pale
skin which juts out to the rough surgical implements of the
world eater legion, metal staples hold his eternally snarling
face to his skull as the ugly "Butchers nails" cortex sticks out
from the back of his head, every pulse from the device bringing
a jolt of pain that makes his face tick every minute or so. His
hair is short and black, mattered in blood that matches the red
of his eyes. His mouth has chunks of his lips missing which peel
back to show his brass fangs, replacing his teeth. His body even
worse as it's patched with healing wounds and scars from various
weapons leaving his skin with torn and ragged scars.
Mental state: He is a savage, having more in common with a rabid
dog than a man, showing signs of memory loss, a common symptom
from the butchers nails but strangest of all when his sleeping
is monitored by psychers they don't come back the same, losing
all feeling in reality.

Enemies: any who stand in his way
Nemesis: me from what I can find
Clothing: simple cloth pants
Armour: A skull and demonic adorned set of Mk 4 armour, blood
stains mix with the black and red cerimite as horns jut out of
the helm and shoulder guard that has a sheet of flayed human
skin adorned over the plate.
Gear: A large two handed chain axe is chained to his wrists
along side his two other smaller chain axes that he wears along
his belt
Owned transport:The death dealer his repurposed prison ship
Personal motives: I'am unsure of his true motive as he moves
towards the ultrama sector he has butchered many worlds and
gathered a sizeable fleet of ships with the men to back it up.
With the ultramarines forces spread out so thinly we are unsure
of how we will defend the sector.
With the power invested in me I, Inquisitor Kraegus, pronounce
him as traitoris Primus and must be destroyed at all costs.

Sexuality: Heterosexual.
Place of Origin: Retrocles III
Approx. Age: 134
Build: Tall, lean,
Allegiance: To the imperium at large, but not the bureaucrats
that run it. To my species by honour, and my friends by duty.
Type/Rank: Lord commissar.
Facial Features –
a) Facial Structure: Strong chin. gaunt cheeks, sunken eyes,
longish face.
b) Hairstyle, colour and facial hair: Black hair, short cut,
typically have stubble.
c) Eyes: Brown, one missing, replaced with green implant
d) Misc: Heavy scaring across his face due to burns. It gives
it a wrinkled, haggard appearance almost like melted wax, and
though fully healed across his body, he tends to wear bandages
across his face and hands.
Body Features: Cirrhotic liver, emphysema in lungs, auto
injector implant fixed to lower stomach. Lots of implants
(imperial design). Generally in an awful state, he has a full
internal organ transplant from a clone every 4 years, one
incoming.
Mental State: Sturdy. Shattered and held together with superglue
and duct tape i.e alcohol and stimms.
Relationships –
a) Romantic: None.
b) Guides: None? I have a ship and crew including navigators
so....?
c) Close friends: Apothecary meros. He's a good man...
marine... uh... organism? Wait is he even that... Either way
he's nice enough.
d) Family (blood): None. Sister (Deceased by his own
plotting), Father (Deceased by his own hand), Mother (Deceased),
Brother (Alive), Clones (Mostly Deceased).
e) Liked persons: The saint (previously muffin), Segovax,
Norabder, (for the most part) Ristiald.
f) Acquaintances: Too many to count.
g) Disliked persons: A lot
h) Strongly disliked persons: A lot.
i) Enemies: Tryzan the infinite...?
j) Nemesis: Maybe Tryzan. Perhaps. But in all honesty he
doesn't really hold grudges that much.
Clothing: Commissars hat, coat, boots, trousers... pretty much
everything in that vein.
Armour: Rosarius, carapace armour.
Gadgets/Wargear: Had a bunch of digital weapons... but I lost
those.
Weapons: Bolt pistols (3), Chainsword, power sword
Owned Transports: The metalus Gravus.
