       1. sql 2005 : sp_lib_helpindex2005
       2. sql 2008 : sp_lib_helpindex2008
       3. sql 2012 : sp_lib_helpindex2012
       sp_lib_ExposeColsInIndexLevels  is called by all the above SPs
