       Unbann Request Template

       Unban requests are for players that have been banned for the
       wrong reason, wrongly accused or for a period of time that seems
       excessive. Unban requests will be resolved by Administrators, if
       the admin directly involved in the banning wishes, they may also
       deal with the request themselves.
       Here is the template:
       Name and Steam ID: Millak, STEAM_0:0:0000000
       Ban Details (Date, Length, Map): Banned for 12 months (A link to
       your ban on Source bans would be helpful) on 7[sup]th[/sup] may
       2012 on the map Trade_plaza
       Ban Reason: I continually swore at and abused other players and
       admins and sprayed up an inappropriate spray while on the
       server, and refused to take it down.
       Any admins online at the time: Stan was online at the time, and
       he is the one who banned me for 12 months
       Why you believe you should be unbanned: I like cookies
