       Ban Request Template

       If you ever see someone breaking the rules in-game and there are
       no admins online to ban the player, please fill out this
       template to help us keep the servers enjoyable for the players.
       The template for a ban request is as follows:
       Perpetrator's name and steam ID: Millak. - STEAM_0:0:000000
       (Make sure the steam ID is exact, copying and pasting from
       console can be difficult so please be accurate.)
       Rules broken: Admin impersonation: Millak was pretending to be
       an admin on tf2 trade#1 today and when I trusted him as a
       middleman, he left the server, taking my unusuals and buds with
       him.
       Time and date of the incident: May 7, 2.10AM
       Evidence: You will need evidence if you want someone to be
       banned, we cannot just take word of mouth from someone because
       we trust them. As evidence we will accept screen shots, and
       demo’s, but others who witnessed the rule breaking confirming
       the rule breaking will also help get the player banned.
