Something Different,
What do you, when talking to elderly PT about previous med.
history they state they have "never been treated for anything".
But when talking to neighbors they state the PT has med history.
What do you do. Is the pt forgetful, are the neighbors wrong?
What do you do?
I forgot to mention, The PT is widower and family lives out of
town.

we can find out alot by using our skills like being observant
for pill bottles or medical equipment.

Maybe also mention a few prominent conditions-i.e. any problems
with your heart, a few minutes later- do you have any problems
breathing, etc. Maybe it will help his memory or he might admit
to one. Maybe denial because he is afraid or worried about cost
of care?

This situation is a judgement call. If the patient is having
difficulty with other questions, then by all means you may
assume that their memory is failing. If the neighbor comes over
and seems as they have a good knowledge of the patient, then I
would go with that information but making sure the I wrote on
the PCR informtion was obtained via a neighbor.

I know I have dealt with this numerous times. Finding a
medication list or medications in the house is a key but also
tell both sides of the story. I have seen this on the rescue and
ambulance side and explain in private with the ambulance crew or
hospital staff that the patient told you one thing and the
neighbor told you different and they can investigate further.
Also if you ask certain questions you may get the right answers.
Like asking about medical conditions and they say none but ask
about medications and they list off some for heart problems and
diabetes.

I would take in as much info as i can from both the neighbor and
by checking out the area around p/t maybe i can notice
medication or other clues .Then i would requestion the p/t

Look around and try to find some indicators and get law enroute
and try to joggle pt's memory a bit.
