       What is the effect on productivity by Self Care Support plan for
        LTC patients?

       There are potential net annual cost savings of £250 million from
       2.3 million more people with a long term condition using
       information, a support group or skills training to better manage
       their condition. Best estimates are that the average cost of
       these interventions would be £125 per person, though this would
       save costs of £244 per person on average.
