Self-Care Support for Long-Term Conditions

Providing patients with long-term conditions with better
information about their disease, choices for treatment and care
pathways, and promoting self care are hallmarks of high- quality
care. Productivity may be improved if long term conditions are
better managed through self care and this results in decreased
medical consultation and hospital attendance.
Organisations where the proposal has been implemented
A wide range of primary care trusts have started to offer a
range of self-care support, including more and better
information; information prescriptions; signposting to self-help
groups; running self-management courses such as Challenging
Arthritis, or the Expert Patients Programme; and are starting to
introduce Telehealth or Telehealth / telecare options to keep
people more independent at home.
