What are the risks involved for Stroke REDS Service?

Taking someone out of a very supportive environment such as a
stroke unit by facilitating early discharge has a number of
risks and if done inappropriately has the potential to have a
negative impact on rehabilitation and decrease quality of care.
To mitigate these following processes are completed, or are in
place, before discharge home:

is suitable for discharge equipment is installed to enable the
enabling care team to assist in personal and daily tasks, and
also to provide adequate support to facilitate independent
functioning at home discharge is facilitated by Stroke REDS who
have a track record of ensuring all risks are minimised as much
as possible before discharge home.
