       What are the barriers to implementation for Stroke REDS Service?

       The Stroke REDS team has identified several operational issues:
       -
       providers where services are not integrated, namely acute stroke
       units with social services, social services with community
       rehabilitation teams and vice versa
       -
       earlier discharges, such as medications not being ready and
       waiting times for investigation being beyond the stroke team’s
       control
       -
       the stroke register is difficult because it relies on
       potentially linking up to three different sources of information
       across three different service providers (acute stroke unit,
       inpatient rehab stroke unit and community).
