What is the evidence of deliverable from implementation of Strok
e REDS Services?

Stroke REDS has:
-
pathway (2009). Currently discharging over 40% of Camden
patients with stroke on ESD pathway (January 2010 to present).
Reducing acute and inpatient stroke unit bed days and, due to
the intensive support and key working offered by the team,
reducing readmission 30 days post discharge. Readmission rate is
under 1% – only one readmission in 3.5 years.

reduced further hospital admissions due to secondary prevention
element of the reviews and actioned appropriate ongoing support
and onward referrals for further rehabilitation to improve
