       What is the impact on patient safety by using Stroke REDS Servic
       es?

       The Stroke REDS team positively impacts patient safety through
       the following:
       -
       belongings, coordinating receipt of discharge summaries and
       onward referrals including outpatient visits).
       -

       package of care to ensure safe functioning at home, including
       minimising falls risks, cutting of keys, provision of basic food
       before the start of an enabling care shopping service.
       -
       person who they can communicate with and access throughout
       rehabilitation with the team.

       including day sitting and night sitting service if required to
       minimise person being at risk while home alone.
       -
       review and update care plans.
       -
       adherence by requesting medication repeat prescriptions.
       -
       community rehabilitation through key working and provision of
       intervention summaries to all relevant stakeholders, including
       referrers at the local stroke units, GP, and patients with
       stroke/family/carers as appropriate.
