The PbR Guidance states that:
The definition of an emergency readmission is any readmission:

and the readmission is equal to or less than 30 days;

readmitted patients when any of these exclusions apply. The

Where system of clinical "failure" may be a cause, research
often distinguishes between "failure" of the healthcare provider
or "failure" of the system (re-enablement, community, primary
care etc.).
To distinguish the two, the time for an Non-Elective readmission
can be used e.g. 7 days. Nationally, over 52% of all
readmissions occur within 7 days which can be attributed to a
"failure" of the healthcare provider. Everything over 7 days is
often attributed to a failure of the system e.g. where support
services are missing or treatment is delayed.
