       What is the evidence of deliverable from implementation of End
       of life service in Community setting?
       The initiative has been implemented in East Berkshire Community
       Health Trust. The project involved 21 trained staff from three
       wards at two community hospitals and most of the professionals
       working at an independent hospice, including nurses, doctors,
       pharmacists, bereavement support and volunteers. The planning of
       the project involved one of the community hospital ward
       managers, a long-term conditions manager, end of life care
       manager, hospice clinical teacher and governance lead and the
       community end of life care tools facilitator.
