What is the impact on patient experience by End of life service
in community?

Significant improvements in patient and carer experience are
anticipated, such as providing care closer to home. Each
hospital group agreed at least two areas for change in their
place of work; the post-secondment sessions provided an
opportunity to identify how these were to be implemented.
Areas identified by staff were bereavement-related issues,
identifying a family room and working with acute and community
hospitals. The final issue was the need for clear pathways and
admission criteria.
