       What is the impact on patients and carers experience by having
       Alcohol Care Team in place?
       Avoidance of admission or readmission to hospital improves
       patient and carer experience.
       A large number of patients will avoid hospital admissions. Just
       as important, a large number of patients will avoid readmissions
       and be placed on a clinical pathway. At present, many have
       frequent admissions and are discharged without their underlying
       problems being addressed.
