The Return of the Player Survey!

Some of you will remember this from the previous message board!
Click here to score the players from 1 to 5 for the last match!
  HTML https://forms.office.com/Pages/ResponsePage.aspx?id=DQSIkWdsW0yxEjajBLZtrQAAAAAAAAAAAANAAaxPuP5UQjYxUzJBQUYzU0NSOVNVUThOMzJNVURaVi4u
The problem with the last survey was it was a major pain typing
       in 23 names every week.
I have a way of doing that semi automatically now, so hopefully
       can do this every week.
Results will usually be published as soon as steam seems to be
running out of updating the survey.
Do you want the subs on the survey?
All suggestions for improvement welcome, but it's a pretty
default setting of MS Forms and a lot I'd like to change (like
using "thumbs up" instead of "Stars") is not possible.
Usual request: Don't vote more than once, this is only a bit of
fun but it screws it up for everyone else.

I think it is quite difficult to score on a 1 to 5 basis.
I would simply look at "okay/ better than average" and "could
       improve".
Basically, even the players who didn't have a great game usually
do a few good things during a match and the good players make
mistakes. But, the okay / not okay makes this easier to score.

       1=could improve, 3=better than average, 5=okay
       Is that better?

Yes, I kind of agonised over words to put to scores of 1-5.
I came up with Poor, Sub-par, Average, Good, Excellent and then
       didn't use them.
I'm a bit loath to drop to a 3 or 2 score survey. A score of Bad
, Average, Good or whatever is a bit too broad a brush to me.
If you score someone as a 2, you can be reflecting their general
play was good but that brain fart of a kick just before half
time was a shocker. Bad says no redeeming features.
But yes, score 1-3 is simpler but 1-5 is more nuanced. What do
       people think?

I like 1-5, as you explain. Enough nuance without being
       overwhelming.

I have a preliminary (possibly final) set of results.
This was 19 responses, which for an away match is reasonable if
I remember numbers in the past.
Obviously many more people see a home match.
In future, I think I'll only do the 15 starter players. The
problem with the subs is if they then play full matches you are
       comparing apples and oranges.
Also I'll remove the player numbers, as any further analysis of
       15. Marcus Smith and 10. Marcus Smith is going to be
       problematic.
But this was just a dry run, thank you to all the contributors.
