That was the crux of the controversial decision at the end of
the NZ Lions series, wasn't it? It's a penalty if C is adjudged
to have played the ball deliberately, but if C just catches it
instinctively a ref will sometimes decide that it was an
accidental offside.
It's one of those areas that probably needs a little tidying up
- I'd probably put the emphasis of the interpretation on whether
they attempt to play the ball. Catch it and stop, or catch it
and release it and I'd regard that as accidental, catch and play
and it's a penalty.
Forward passes are a bit of a carve-out from the general offside
rule. For one thing, it's legal to catch a flat or backwards
pass from a position in front of the passer as long as you are
reaching back to get it (as illustrated by the Ellie Kildunne
try on another thread).
For another, if we applied offside strictly on a forward pass
under TMO review, then an offside line would be created at the
forward pass which could mean a penalty for anyone who handled
it after that. But in practice that would create both chaos on
the pitch and far too big a penalty for the offence, so we
handle it a bit differently.

A forward pass is defined by it looking wrong to the referee.
In 99% of the game, that is the only option, so that is what we
need to use.
Which means that you are much more likely to be pulled up if you
pass close to a white line, which some people felt was unfair.
But I find the current phasing to be just as problematic.

Well, heres a coupla suggestions:-
1. Scrap the Forward Pass Law altogether.
2. Institute a 5 or 10 yard zone as a margin of error for the
recipient (as long as he starts his run behind the despatcher)
3. Change the markings on the pitch to maybe have regular dotted
lines width ways to aid the recognition of a forward trajectory
from despatcher to recipient.
4. As players & coaches will naturally push the boundaries for
any little advantage amend the Laws so that even if a player is
running at pace he HAS to offload/pass backwards. Zero
tolerance.
5. Look at the ruck laws properly so the subsequent D issues
don't become a problem. Unintended consequences etc.

Remember that if you take a forward pass, you are not offside.
If the forward pass law was not there, there could be no
accidental offside.
That would lead to many more offside penalties.
