These sort of issues in rugby are annoying.
From reading through social media, etc, it looks as though the
ref had two options:
1. play on
2. red card
Both could probably have been valid options.
It is like the case where a player tries to intercept and ends
up bashing the ball down instead.
If the ball goes directly downwards - then play on.
If the ball goes an inch forward, then yellow card and penalty
try.
Or, a full back jumps to catch a ball.
One player from both sides goes running in.
Own player hits the full back in the air - no sanction.
Opposition player hits the full back in the air - potential red
card.

Add to that the use of outcome as an influence on sanction.
Someone aiming to cause injury but failing, may go unpunished.
Someone accidentally causing a potentially bad injury could be
red carded.

The irony is that if Steward had simply carried on advancing
straight on the injury to Keenan might well have been more
serious but in all probability it would have been judged a
collision

Please could you provide examples of where someone messes up
badly, accidently injures an opponent and gets sent off?
As there are "lots of sports" where this is a red half a dozen
or so incidents should be easy to list

Sure. I've seen many red cards given for a player badly messing
up an attempt to challenge for the ball in the air, leading to a
scenario where the opposition player has been tipped
dangerously. I'm sure you have too. Bad tackle technique in
rugby leads to reds all the time - someone tries to do
something, does it incompetently, and gets a red because they've
ended up whacking someone in the head.
A mistimed tackle in football can lead to injury and a sending
off.
I'm not entirely sure what you're asking for - are you actually
arguing that this isn't the case? What do you think most reds
are given for? Deliberate foul play?

What I was asking for was examples of the lots of sports where
your scenario played out. Going for a catch in rugby seems to be
one. But you came up with no other real examples- names and
places. Most red cards are for deliberate acts or attempted acts
e.g. as you pointed out jumping for a ball.
I understand that you think that the red card was correct but
your justification based on the "lots of sports" argument was
what I was questioning as it seems somewhat stretched

You seem to have dramatically misunderstood what I said, then.
At no point am I talking about pure happenstance - none of this
is accidental in those terms. It's people attempting something
and messing up badly.
Almost every contact sport has the equivalent of attempting
something legal but messing it up so badly it ends up illegal,
dangerous, and resulting in a red card or equivalent. It's a lot
more common than deliberate foul play - and me not listing them
exhaustively is because it's self evident!

Here you go Jammy - Football - Wales v Iran - Goal keeper Wayne
Hennessey tries to kick the ball, but kicks the Iranian player
by accident = red card

Yeah, keepers are prone to it! Go to punch the ball and get an
opponent instead and it's often a red...
Some very famous examples of making a huge mess of it and
getting away with it, too!

I worry (But would like to think not, as its not football) that
players might look to exploit refs being panicky with anything
to do with the head. If you know anything connected to a head
collision would probably end in a red, is it a gamble worth
taking to get you an advantage in a crucial game?
