According to someone on the Sarries board, KD asked if it had
occurred in this phase, meaning since the last restart, but his
use of the word phase confused the TMO so she said no.
I can't corroborate this as I didn't watch the match.

Yes this is about right, He asked was it more than 1 phase ago,
TMO said Yes, then He said He couldn't review ...play on

I read that it was the TMO's first time in that role. That said,
it was the ref's misuse of the word phase that caused the
problem. He should have asked since the last restart or start of
play.

The TMO was weak and Dickson clearly doesn't know the protocol.
The sport deserves better.
