Ban Appeal Format [READ BEFORE POSTING]

If you got banned recently and you're willing to have an other
chance to play in the server, create a new topic in the 'Ban
Appeal'-Section (this section) on our forum AND use the
following format to do so. The correct answers to these
questions are required to process your appeal quickly:

Why were you banned? Write here the reason stated by the admin.
Who banned you? Write here the name of the admin who banned you.
Why should you be unbanned? Explain why you should be
unbanned.[/code]
Do you believe that this ban is incorrect and that you're
innocent?: If so, please post screenshots or videos that prove
that you're innocent, etc...
Your account may have had any of the subsequently stated bans.
It is essential your read upon the following bans to ensure you
understand whether your ban is applicable for an appeal. Please
contact the Management, if you have any queries or doubts about
your ban that can not be resolved in an appeal.
Temporary Ban - A Temporary Ban is a ban which prevents you from
logging on for a set time-frame. These bans are applied usually
for the violation of minor server offenses however appear to be
a recurring issue. You may not appeal a temporary ban unless you
believe it was wrongly given.
Permanent Ban - A Permanent Ban is not temporary. However is
applicable to be appealed if you wish to request a second chance
on our server except in some rare occasions when the Management
say the opposite. It is assigned to your banning Administrator
and only they, or someone superior may handle it. They reserve
the right to deny it at their discretion.
