How do you deal with forum spam?
I use security questions, but I also require admin approval on
all my forums, as well.
How about you all?

I think on here, it's a box to click on to prove your a human
and some kind of captcha

Well, sometimes the security question is too easy, like "this
forum is called blank blank blank. What's the middle word in the
name?" Or, "type 'agree' if you agree to follow rules of this
forum." I think we can do better than that without getting too
difficult.

I always try to keep my security questions niche-relevant.
If I had a forum about peanut butter, for example, I'd ask a
question like "Where was Peanut Butter invented? Two words" The
answer is "Montreal Canada" This makes the questions somewhat
difficult. AI bots would likely get right past it though.

I have phpBB. I ask new users to write words backward at user
registration. It keeps out nearly all spam, but not all.
Ex: What is sun spelled backward? Answer: nus

I installed a phpBB, MyBB, and SMF last year just to play with
them since it had been a long time since I used them.
I found phpBB to be the most featured, but I had so much spam on
that compared to the other ones. I used a tough security
question, as well.

Yeah. That's weird. I guess spam doesn't come at everyone at the
same rate, even with tough prevention.
I did notice on a forum I had before that hanging around certain
forums where my signature was present caused a high influx of
spam to come in.
