Since you are asking, I think it’s creating drama to refuse the
exterminator entry if you actually want the problem solved.
I think that refusing a call from the landlord under the guise
of “too sick to answer” and then calling the health department
falls under the same category.

It may seem fishy to some people but he has a history of hiring
people who may have criminal backgrounds and has possibly
"hired" someone to chase out another renter who was a member of
a group of people he did not approve of. -He has been arrested
for some crazy stuff too. I don't trust him nor do I trust him
to hire trustworthy people.
We were supposed to have a handy man who cut the grass and
shoveled for us at the owner's expense and that person never
materialized. Some of our neighbors have told me that my
landlord hung out with people and hired people who liked to use
the same illegal substances that he likes. That sounds like
something he would do. He has done a lot of "work" on the house
himself and I can't imagine any of it is up to code. He is a
major cheapskate. He decided to use a cut rate person to fix the
heating system and they never fixed it. They may not have been
licensed either. Already his "pest control person" according to
one of the roommates says that either we tenants caused the
infestation or trees touching the house did which the pest
control guy wants to be hired to trim.
I think we have a species known as "little black ants" infesting
our house. They like damp places like the kind of place that has
black mold and mildew all over the basement and look like the
pictures in google and I have seen flying ants on a cyclical
basis as little black ant species are known to do in North
America in July and August.
I decided not to call him again tonight because he feeds on
drama and I don't honestly know what I will say to him. None of
us trust his representatives given his track record. I want to
be given a license number and a company name, 24 hours written
notice and a description of what the inspection will entail.
I sent him a much shorter email explaining need for 24 hours
notice and what that notice should entail and I also thanked him
for speaking with someone in pest control to finally start the
process.

What I need help with is how do I manage to live with this
person for 2 or more months with him without things getting ugly
while I still live here?
These are probably not the right moves to keep things calm.
These things just up the drama...
He tried to call me but I was too sick to answer.
So I called the health inspector on him and made a complaint.
At least one of my roommates is angry with me right now because
I called the health inspector.
I decided not to call him again tonight because he feeds on
drama and I don't honestly know what I will say to him.
If you want the ant problem solved, don't drag it out. Let the
guy come solve the problem. Also answer the phone and talk to
him.
Find another place as fast as you can, the stress of this can't
be good for you.

Then you shouldn't be renting from him. You should move
somewhere else.

It seems like you need legal advice.
I do not believe a tenant is entitled to be provided with a
license number and description of work to be performed when the
landlord hires someone to perform work on their own property.
Also, even people with “possible” prior criminal records need to
feed themselves and do in fact often obtain legitimate jobs
doing home maintenance and repairs.

The health department told you that he would be sending a
professional. They aren't going to accept documentation from
just anyone in regards to your complaint.
Your roommates may be upset with you because they don't want the
health department shutting the place down. It might be infested
with ants but it's affordable for them. Maybe to them having
ants around is better than a lot of other problems that a rental
in that price range could have.
You do have rights as a tenant. But a court is not going to
sympathize with you not letting in a professional (mandated by
the health dept) because you didn't get enough notice.
Something to consider.

I do think that the court would understand that she wants to see
some kind of proof that the "exterminator" is a licensed
professional before letting *random stranger* who claims to have
been sent by the landlord but has no documentation of being
trained to perform the inspection or spray the place with
poisons...
I have heard of an entire family getting very, very sick when
someone "treated" their house with the wrong poison by mistake -
it was on the local news some years ago (we didn't live in the
same place - it turned out that the containers were the same
color and someone picked them up without reading the name of the
product, as I recall).

But, the reason he is sending the exterminator is because she
called the health department. He has to send a real
exterminator because he promised a government agency that that's
what he was going to do. He can't just send any random person.
Her "proof" is the health department telling her that.
I can understand not trusting him if there was no outside
pressure to mandate him to hire a professional but that's not
the case.

And with the landlord's history of sending random people he met
recently - getting ID to confirm that it's a licensed
exterminator from a local company is probably not a bad idea. I
remember being told that I couldn't steam clean my carpet when
moving out of a rented apartment, I had to hire someone from a
short list of "professional companies" that the hiring office
had. Then I had to turn over the receipt as "proof" - even
though the rental agent showed up early, while the guy (wearing
the company shirt) was still there, finishing up the last room
while she "inspected" the rest of the place. She was going to
charge me the cleaning fee (the carpet was damp & she'd watched
him finish & then take apart the equipment) because I hadn't
handed her the receipt yet.
I finished signing the paperwork with the carpet cleaner &
handed her the blasted receipt. Did not understand why the
evidence of her own eyes wouldn't let her hold off while the
paperwork was finished - but...I made sure that I'd cleaned
under the fridge, stove, dishwasher, and the various cabinet
shelves & drawers. In my personal opinion (based on the candy
wrappers, small toys, and Christmas ornaments that I pulled out
from under the appliances) - the place was a lot cleaner when we
moved out than when we'd moved in...but you can only do so much
when the vinyl flooring really needs replacing, not cleaning.
