I have a very good (but unexciting) update! Met Mary for a
coffee yesterday and deployed my strategy of letting her pay
first. She paid for both our coffees (on her card) with a
cheerful "You shouted me before, so it's my turn this time."
So no further action was necessary!

I'd go with reminding her beforehand to get cash out. Your own
suggestions are fine on how to do that.
Next time ask if she has cash as you get in line, and make a
point of going back out to an atm. Or have her order first and
when she finds no money have her use her card by reminding her
"it's a 10$ minimum? Well how about, as I covered you last time,
we add mine to your bill and you use your card?"

Check out the update at #50 above. She basically did what you
suggest in your last line, except the friend offered first.
