Okay, I have it. It's from grade four and totally ridiculous:
Mr. Smith had 565 jellybeans. 312 jellybeans were raspberry
flavour and 25 were blueberry flavour. How many jellybeans was
lemon flavoured? Use all strategies to help you sole this
problem.
1. They left out details. The reat are lemon but they didn't say
so
2. Terrible grammar - "How many jellybeans was lemon flavoured?"
3. It's so easy. I can't believe that was in grade 4. I'm not
kidding. No offense if my teacher wrote this, but it was just
not our level.

Here's another:
Eight dolphins each have the same mass. Their total is about
1195kg. What is the approximate mass of each dolphin?
Again, no offense, teacher, but it was way too easy for us.
Last of the Top Three Worst Problems:
Mr. Has-a-lot received 4 boxes with 45 chocolate bars inside of
each box. Miss Wants-a-lot wants the same amount of chocolate.
She already has 65. How many more would she still need to equal
Mr. Has-a-lot's amount? Use all strategies to solve this
problem.
Terrible names and way too easy.
