Port the Synchronet (Novice) command shell from Baja to JavaScript

simple.js replaces simple.src/.bin, completing the port of the stock
command shells from Baja (only matrix.src and ftp.src remain in exec/).
Written fresh from simple.src rather than from the abandoned draft in
MR !698.

Same menus (text/menu/simple/*) and the same word commands typed at the
Command: prompt, including the Send, Read and File sub-menus. Anything
unrecognized at the main menu is still passed to the string commands,
with no ';' prefix. Each command maps to the equivalent shell_lib.js
function or bbs method rather than a reimplementation.

The shell's "{Continue? Yes/No}" pause prompt is now reverted when the
user leaves the shell, rather than staying in effect after a switch to
another shell.

Since a shell's exec/<code>.js takes precedence over mods/<code>.bin, a
sysop's customized mods/simple.bin (or mods/sdos.bin) is no longer used;
noted in the v3.22 release notes.
