"frequently asked questions" turns into:
"con frecuencia hechas preguntas"

and turns back into:
"frequently made questions"

That's no good. How about "questions asked frequently"? That turns into:

"las preguntas pidieron con frecuencia"

and turns back into:
"the questions requested frequently"

That seems pretty good.
